What to Bring

Please bring the following with you to your first appointment:

  • Driver’s License or Valid ID
  • Insurance Card
  • Referral Letter (if applicable)
  • X-Rays, MRIs, CT Scan images in CD format as well as reports
  • List of Current Medications
